
Welcome to Mindful Living of Central Florida! I feel that being totally transparent in our therapeutic relationship is critical. This is why I have created this document, and it is very important that you read this thoroughly and asks about any questions you may have. If after reviewing these things together either of us feel that your needs cannot be met at Mindful Living of Central Florida, LLC a referral will be made to a provider that can adequately meet your needs.
HOURS OF OPERATION
Monday through Thursday from 4:00pm-8:00pm
Saturday: 8:00am-12:00pm
Clients are seen by appointment only
CANCELLATION OF APPOINTMENT AND NO-SHOW POLICY
Keeping scheduled appointments is a large factor in how successful therapy will be. If you are not able to attend a scheduled appointment, I request that it is cancelled at least 48 hours in advance by calling (407) 900-1516. Please leave a message if I do not answer. My policy is that 3 late cancels (less than 48 hours’ notice, 3 no show, or any combination of these will result in being discharged from services for 6 months. A discharge letter will be sent to the address in our records. I, in return, will always attempt to give at least 48 hours’ notice if I need to cancel due to emergency. I will also do my best to reschedule your session for another time the same week if possible.
OBTAINING RECORDS
Patients, parents, or legal guardians may obtain copies of their records by signing a Release of Information Form. I do require a 10-day turnaround time for this.
FINANCIAL POLICY
(Please review the Financial Policy you are given thoroughly)
Any costs for treatment will be paid privately for the length of the therapy session, and whether it is an individual or group session. As the financial policy states, all payments are due at the beginning of the session, as well as any balance you may have, or the session will not occur. Continued unpaid balances may result in termination of services.
CONFIDENTIALITY

All records and client information are kept confidential in compliance with HIPPA, as well as ethical standards set forth in my profession. Your information will not be released unless a situation discussed in the confidentiality handout applies.
SERVICES AFTER HOURS
In the case of an immediate, life-threatening emergency after hours please call 911. If you feel you need to speak with a mental health professional, please contact: Community Crisis Line (407) 425-2624 Teen Hotline (407) 841-7413. If you have attempted at least two of these options, and are still in crisis, you may contact me at (407) 900-1516. I will attempt to schedule you ASAP (preferably the next day).
INVOLUNTARY DISCHARGE FROM SERVICES
Issues related to attendance, non-payment of services, extremely disruptive behavior, violence, or non-compliance with treatment recommendations may lead to involuntary discharge from services. If this happens, you will have 30 days to find another provider (which I will attempt to assist you with), and you can request information on how to file a complaint with the State of Florida if you decide to do so. Threatening or abusive behavior toward any staff or other clients in the building may result in immediate termination of services.
